Hello! Welcome to your patient portal. Please ensure the below steps are complete to ensure a smooth transition into Dr Baker's new private practice:
- Upload an updated credit card: For security purposes, all previous card/payment data was unable to be transferred. Please update your credit card information under the Invoices and Payments tab.
- Upload a picture: If you do not have a picture in your chart, please upload one by clicking on the My Account Details tab. You can also double check that your address and contact information is entered correctly.
- Ensure all consent forms are completed: Go to the Forms and Questionnaires tab and make sure all forms are filled out. Ensure they turn green (indicating completion) vs red.
- Feel free to message me through the Secure Messaging tab if you need assistance with anything!
